Dear Marsiling DC,
I had twice, cannot be forgotten, haemodialysis (HD) sessions treatment or therapy this week. Down below is just my brief summary being extracted from daily rough note-book.
1) On Wednesday, 20th October 2021
Temp 36.2 C
My Pre weight 65.7 kg
Dry weight surprisingly set at 63.6 kg without my knowledge
Diff 65.7 - 63.6 = 1.9 kg
Setting target goal 2.2
Stopped water supply for 10 minutes; pump speed reduced to 230 due to uncomfortable feeling...
2.2 downed to 2.0, then again down to 1.8 & finally only managed to remove about 1.6 kg of fluid as I requested to finish earlier as I couldn't tolerate anymore!!!

Standing BP 142/54
Post weight 64.3 kg ???
At/V (?)
Had a small cup of warm drink
Being advised earlier to have someone accompanied me going home.
Remark/comment:- I was feeling uncomfortable & this is very unusual case indeed! It was adjusted or reduced to 2.0 kg. Again later on, I felt difficult to breath & need oxygen to support in addition my body so stressful with shivering too. Very tired! Had a cup of warm plain drink & my 2nd son fetched me home. Luckily we managed to timely taking appropriate remedy action as a team to avoid any unforeseen circumstances arising. Thanks! Apart from very tired I was okay & fine. FYI, I had stopped taking BP tablet (Telmisartan ~ 20 mg a week ago except Sunday during none dialysis days when BP slightly on high side as per my BPs records list shown ~ as instructed earlier too whenever my BP is low at home).
Home BPs:-
a) 5.55 pm 116/67 ~ 85 ; 112/65 ~ 80 with old BP device
with new BP device 109/67 ~ 82 ; 105/72 ~ 89 before dinner.
b) 7.15 pm 97/51 ~ 96 ; 105/55 ~ 89 ; old type of BP device
New BP device 102/62 ~ 76 ; 103/66 ~ 66

2) On Friday, 22nd October 2021

Diff 66.1 - 63.9 = 2.2 kg
Setting target goal 2.5 kg
Remark/comment:- This time I felt uncomfortable much more earlier than the Wednesday HD session. It was within the first one & half hour only. Pump speed reduced to 280. Target reduced to 2.2 kg, then again 2.0 kg due to very uncomfortable & intolerable state. It was similar like Wednesday symptom too. Difficult to breath + shivering too. Because of the machine pulse indicating so low, the senior nurse kindly & timely checked my heart-beat or pulse manually & it was okay with 54. Finally I finished earlier too with achieved target only at 1.811 kg.
Post BP sitting 157/68 ~ 80
Post BP standing 129/57 ~ 80 ; feeling good & fine.
Post weight 64.4 kg & At/V 1.55.
At home,
I felt okay & fine as normal state despite what had been happening today. So I decided to go back home myself through public transportation with mindfulness movement throughout my journey home. A bit tired though! Had a hot drink & a bite of snack upon arrival home. Again it is good & nice we work as a team at centre to timely make remedy or correction whenever or wherever necessary from time to time or case to case basis. Cheers!

7.15 pm 96/60 ~ 71 ; 102/60 ~ 86 using new type of BP device
Using old type BP device 111/52 ~ 94 ; 118/66 ~ 90 ; noticing my pulse on high side & taking my medication.
Personal conclusion or summary as a dialysis patient:- Perhaps it is right timing to study & make a fresh re-evaluation on my personal individual HD session treatment in respect to the formula or program. Also, I like to raise up the body-composite-measurement once again. It had been a year plus since my last dry weight being increased from 63.3 kg to 66.5 kg sometime in September 2020. That was a sudden big jump indeed! Can we possibly arrange a body-composite-measurement checking or testing somewhere in NKF DC? I am willing to travel to make the checking or testing if recommended!
